* Reflecting - Both classes of Stock Appear on the face of each Certificate with a Total Authorized issue in the Center of the Certificate. The Certificates are typically differentiated by color, prefix & the fully paid line (i.e. fully paid and non-assessable shares of Common Stock). There is a multi-class fee $10.00 per Class. 10 Certificates per Class are included with this order (20 Certificates Total)**You may elect to use the 20 Certificates in any combination (i.e. - 20 Certificates for the 1st Class, No Certificates for the 2nd Class).

* Non-Reflecting - Only 1 Class will be placed on each Separate book of 20 Certificates (40 Certificates Total). There will be no Total Authorized issue listed on the Certificate Books (this would not apply since the shares are listed on separate Stock Certificate books). The Certificates are typically differentiated by color, prefix & the fully paid line (i.e. fully paid and non-assessable shares of Preferred Stock). There is an additional charge of $36.00 per book of 20 Certificates. One book of Certificates are included with the Kit. Total Additional Charges $36.00. **You may elect to use the 40 Certificates in any combination (i.e. - 30 Certificates for the 1st Class, 10 Certificates for the 2nd Class).

Order Form Instructions
* When filling out this order form a "*" to the left of the option indicates Required Fields listed below.
* Type the Name of Incorporation exactly as you wish it to appear on the Stock Certificates.
* Mo. Day, Yr. required for CA and HI only; all other States Year only applies.
* When typing Total Authorized Shares use numbers only (i.e. 10000).
* When typing Number of Shares use numbers only (i.e. 10000).
* Signature Titles are listed in ranking order of the officers of the Corporation. The highest ranking officer is placed on the bottom right of the Certificate, followed by bottom left, top right and top left.
